A Level Four Independent Living Program (ILP)

Community Apartments Model

Connections ILP is an 8-bed Independent Living Program designed for young adults ages 18-20 who have demonstrated the maturity, responsibility, and self-regulation skills necessary to live successfully in the community with minimal supervision. Unlike traditional residential programs, Connections places participants in leased apartment homes throughout the community, providing a realistic and supportive environment where young adults can practice full independence while continuing to receive guidance and support from CCYA staff.

The program represents the final step in CCYA's continuum of care and is intended for residents who have successfully progressed through the organization's Transitional Living Programs. Participants are expected to demonstrate strong decision-making skills, personal accountability, financial responsibility, and a commitment to educational, vocational, and employment goals.

Eligibility and Program Expectations

To be considered for Connections ILP, applicants must:

  • Be at least 18 years of age.
  • Successfully complete a minimum of three to six months in Rise Up Studios while demonstrating readiness for independent living.
  • Receive a written recommendation from the Rise Up Studios Program Director.
  • Have the approval of their assigned DFCS Caseworker.
  • Demonstrate consistent responsibility, communication, and self-management and regulation skills.
  • Maintain a drug-free lifestyle and participate in monthly drug screenings while in the program.
  • Be enrolled in school, a vocational training program, workforce development program, or post-secondary education and maintain part-time employment; OR be employed full-time.
  • Participate in ongoing therapeutic counseling services.
  • Meet regularly with assigned CCYA Staff.
  • Provide proof of school enrollment, attendance, and achievement.
  • Provide proof of employment and a bank account.
  • If a car a is owned, must provide a current driver's license; proof of annual auto emissions test and registration; and proof of monthly insurance payments and coverage.

These expectations are designed to ensure (a) safety; (b) legal compliance; (c) and preparedness for the increased freedoms and responsibilities associated with independent living.

Supportive Housing Services

While residents enjoy a greater degree of independence, they continue to receive comprehensive support services, including:

  • Fully furnished apartment accommodations
  • Rental and utility assistance (CCYA serves as the guarantor on the lease with the resident)
  • Monthly resident stipend in the amount of $300
  • Individualized Service Plans
  • Case management and care coordination
  • Life Skills Education and independent living coaching
  • Educational enrollment support and financial aid assistance
  • Employment placement assistance
  • Career exploration and planning
  • Financial literacy and budgeting support
  • Continued access to medical and dental care
  • Individual counseling and behavioral health support
  • Ongoing drug screening and accountability services
  • Basic needs assistance and resource coordination

Building Lasting Independence

Connections ILP provides young adults with the opportunity to apply the life skills they have learned in residential care while navigating the responsibilities of adulthood in a real-world setting. Participants manage their own schedules, transportation, employment, finances, household responsibilities, and personal goals while benefiting from the safety net of continued professional support.

The program's ultimate goal is to help young adults successfully transition into permanent housing, stable employment, higher education, military service, or vocational careers while developing the confidence and skills needed for long-term self-sufficiency.

Program Note: Connections ILP apartments are typically two-bedroom units shared by two residents. Apartment placement is dependent upon the availability of eligible participants and suitable housing opportunities within the community.
